bbc429148f Add a note that generated files are generated.
Folks keep assuming checked-in assembly files are the source. Between
the preprocessor, delocate, NASM not using the C preprocessor, and GAS's
arch-specific comment syntax, comment markers are kind of a disaster.
This set appears to work for now.

583c12ea97 Remove filename argument to x86 asm_init.
43e5a26b53 removed the .file directive
from x86asm.pl. This removes the parameter from asm_init altogether. See
also upstream's e195c8a2562baef0fdcae330556ed60b1e922b0e.

3e700bb3e8 Get MASM output working on Win32.
We were building the NASM flavor with MASM which is why it didn't work. Get the
MASM output working: cpuid and cmove are not available in MASM unless the file
declares .686. Also work around MASM rejecting a very long line in SHA-256.

The follow-up change will get the NASM flavor working. We should probably use
that one as it's documented as supported upstream. But let's make this one
functional too.
