be941d4659 Correctly find all critical CRL extensions.
Unhandled critical CRL extensions were not detected if they appeared
after the handled ones.  (Upstream GitHub issue 1757).  Thanks to John
Chuah for reporting this.

This additionally adds a regression test for this issue, generated with
der-ascii.  The signatures on the CRLs were repaired per notes in
https://github.com/google/der-ascii/blob/master/samples/certificates.md

54afdab584 Fix X509_CRL construction under malloc failures.
If asn1_item_ex_combine_new fails in one of the ASN1_template_new calls
just before the ASN1_OP_NEW_POST call, ASN1_item_ex_free will free the
temporary object which ultimately calls ASN1_OP_FREE_POST. This means
that ASN1_OP_FREE_POST needs to account for zero-initialized objects.

29270dea85 Split unlock functions into read/write variants.
Windows SRWLOCK requires you call different functions here. Split
them up in preparation for switching Windows from CRITICAL_SECTION.

57707c70dc OpenSSL reformat x509/, x509v3/, pem/ and asn1/.
OpenSSL upstream did a bulk reformat. We still have some files that have
the old OpenSSL style and this makes applying patches to them more
manual, and thus more error-prone, than it should be.

This change is the result of running
  util/openssl-format-source -v -c .
in the enumerated directories. A few files were in BoringSSL style and
have not been touched.

This change should be formatting only; no semantic difference.

6fb174e564 Remove last references to named locks.
These ASN.1 macros are the last references to the old-style OpenSSL
locks that remain. The ASN.1 reference count handling was changed in a
previous commit to use |CRYPTO_refcount_*| so these lock references were
unused anyway.

4bdb6e43fa Remove remaining calls to the old lock functions.
|SSL_CTX| and |X509_STORE| have grown their own locks. Several static
locks have been added to hack around not being able to use a
|CRYPTO_once_t| in public headers. Lastly, support for calling
|SSL_CTX_set_generate_session_id| concurrently with active connections
has been removed. No other property of an |SSL_CTX| works like that.

054e682675 Eliminate unnecessary includes from low-level crypto modules.
Beyond generally eliminating unnecessary includes, eliminate as many
includes of headers that declare/define particularly error-prone
functionality like strlen, malloc, and free. crypto/err/internal.h was
added to remove the dependency on openssl/thread.h from the public
openssl/err.h header. The include of <stdlib.h> in openssl/mem.h was
retained since it defines OPENSSL_malloc and friends as macros around
the stdlib.h functions. The public x509.h, x509v3.h, and ssl.h headers
were not changed in order to minimize breakage of source compatibility
with external code.

b06711ca34 Move the X509_NAME typedef into x509.h.
X509_NAME is one of the symbols that collide with wincrypt.h. Move it to x509.h
so libraries which only use the pure-crypto portions of BoringSSL without X.509
needn't have to resolve the collision.

- OPENSSL_NO_RC4
- OPENSSL_NO_RSA

Also manually removed a couple instances of OPENSSL_NO_DSA that seemed to be
confused anyway. Did some minor manual cleanup. (Removed a few now-pointless
'if (0)'s.)
